The KDCHC Primary Care Outreach Team is a team that provides primary care and referrals to people who may experience barriers to accessing health care, especially those who have experienced or who may be at risk of homelessness. 

The team can assess your physical or mental health concerns and offer treatment or a referral, and will support you without judgement and from a harm reduction lens. 

*A health card is not required, but the team is equipped to help you get one if you do not have one.
